📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, balance, strength, endurance, and safety.
• Implement wound care and evidence-based dressings.
• Develop and adjust individualized care plans and PT interventions.
• Document observations and evaluations, coordinate with nursing team.

🎯 Requirements

• North Carolina Physical Therapist license.
• Graduate from an APTA/CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with electronic medical records (EMR) and tech tools.
