Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients
• Develop a treatment plan based on evaluation, physician orders, and goals.
• Continually assess and revise the care plan and participate in multidisciplinary care.
• Educate patients and family on rehabilitative care to promote health and independence.
• Document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in VA.
• Certificate of Clinical Competence in Speech from ASHA (CCC-SLP) or equivalent credentials; or be
• Completed education and experience requirements for certification or ongoing supervised experience.
• Solid computer skills; EMR experience preferred.
