📋 Description
- Support high-acuity and medically complex populations in care and recovery journeys.
- Combine population health management, care coordination, data analysis, and direct member
- Focus on pregnant/postpartum members, recently hospitalized individuals, and those with complex
- Partner with medical providers, counselors, specialists, and care teams across regions within a
- Identify care gaps, monitor member progress, coordinate interventions, and contribute to quality
- Contribute to pilot programs and improve care delivery for vulnerable populations.
🎯 Requirements
- Current Registered Nurse (RN) license; Maine LPN with SAMHSA X-DEA training may be eligible.
- Bachelor's degree in Nursing preferred.
- At least 5 years of direct patient-care experience in addiction medicine, behavioral health, or
- Strong care coordination, population health, and interdisciplinary delivery knowledge.
- Understanding of HIPAA and healthcare regulations, professional conduct standards.
- Clear communication with dispersed teams and diverse member populations.
