📋 Description
- Pharmacist embedded in Rhode Island primary care practices within RIPCPC ACO.
- Focus on COPD population health: transitions of care, med optimization, inhaler education
- Provide evidence-based, patient-centered med management to improve chronic disease outcomes.
- Collaborate with interdisciplinary care team to reduce avoidable healthcare utilization.
- Support ambulatory care services and quality initiatives as needed.
- Participate in program development, quality metrics, and reporting of outcomes.
🎯 Requirements
- Providence or RI licensure required; unrestricted Rhode Island pharmacist license.
- Board certifications or ASHP residency in ambulatory care preferred; PharmD degree required.
- Experience with COPD/respiratory care, inhaler technique education, and population health.
- Experience under a Collaborative Practice Agreement (CPA) and independent medication management.
- Strong documentation, EHR usage, and interprofessional collaboration skills.
- Knowledge of chronic disease management, transitions of care, and quality improvement.
