Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and other client files.
• Provide ongoing life skills development to improve independence and quality of life.
• Communicate effectively to document care, follow support plans, and collaborate with internal and
• Provide targeted interventions when clients are at risk of eviction.
• Connect clients to medical, mental health, and other supportive counseling services.

🎯 Requirements

• Bachelor's Degree in social services or related field with at least one year of housing
• Ideal candidates have self-starting attitude, strong time management, and proactive problem-solving.
• Flexible schedule, tech-savvy (Microsoft Office, Zoom, PC, Android).
• Valid driver’s license, insurance, and access to a vehicle.
• CPR/First Aid ability; ability to lift up to 50 lbs; must be at least 21; bilingual preferred.
• Strong reading, writing, and communication skills for documentation and collaboration.

🎁 Benefits

• Wage: $23.00/hr; full range $23.00 - $25.65 per hour (based on tenure and performance).
