📋 Description

• Perform diagnostic tests and measurements in home settings (mobility, transfer, balance, gait
• Develop and implement individualized care plans (manual therapy, gait training, balance, other PT
• Provide integumentary care to prevent wounds and promote healing using evidence-based dressings
• Continuously assess and revise the PT care plan
• Educate clients and families in rehabilitative care to promote health and independent living
• Document observations and interventions using a touch pad tablet

🎯 Requirements

• Current Physical Therapist license in Hawaii
• Graduation from APTA- or AMA-accredited program
• Solid computer skills; EMR experience preferred
