📋 Description

• Telehealth-enabled care coordinator within Advanced Care at Home
• Supports patient throughput, care coordination, and longitudinal case management
• Operate under RN/provider supervision and within licensure scope
• Coordinate day-to-day care activities across a virtual patient population
• Escalate appropriately when needed

🎯 Requirements

• Active LVN/LPN license in good standing. CA license preferred
• 2+ years clinical experience (acute/home health/case management)
• Experience with EMR/EHR systems and clinical workflows
• Strong telephonic/virtual communication skills
• Ability to manage multiple priorities in remote setting
• Demonstrated ability to work within scope and escalate
