📋 Description
- Make home visits to clients in Abington and nearby areas.
- Assess mobility, ROM, balance, gait, strength, and safety.
- Implement wound care and protective interventions with evidence-based dressings.
- Develop and revise individualized care plans; include manual therapy and gait training.
- Educate clients and families on rehabilitative care and safety.
- Document observations and interventions using a touchpad EMR system.
🎯 Requirements
- PA Physical Therapist license.
- Minimum one year PT experience.
- Graduate from an APTA/CAAHEA approved program.
- Ability to work independently and manage time.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; EMR experience preferred.
