📋 Description

• Start of care home health visits in Monroe County
• 1:1 care with flexible schedules
• Home visits for patients in designated territories
• Follow care plans and document with EMR touchpad
• Coordinate case management and doctor orders

🎯 Requirements

• PA RN license (current)
• At least 1 year clinical experience
• Home care experience preferred
• Graduate from accredited nursing program
• Ability to travel to cases
• Strong interpersonal and time management skills
