Job Description
📋 Description
- The Physical Therapist provides evaluation and treatment across acute, restorative, and maintenance
- Develops individualized treatment plans and conducts home visits as needed.
- Educates participants and caregivers on exercise programs and DME use.
- Contributes to the Interdisciplinary Team (IDT) with assessments and care coordination.
- Maintains documentation meeting regulatory requirements and supports program standardization.
🎯 Requirements
- Bachelor’s Degree in physical therapy
- Active California Physical Therapist license
- Current CPR/BLS certification
- Excellent communication with participants and caregivers; documentation and care coordination skills
- Experience with documentation, regulatory compliance, and safety protocols
- Preferred: 2+ years as a practicing PT; bilingual Spanish/English strongly preferred
