📋 Description

• Collaborative Care Clinician I provides care for pediatric and/or adult patients.
• Uses evidence-based interventions to reduce symptoms.
• Works with Care Pod to customize patient treatment plans.
• Collaborates with PCPs and other providers for integrated care.

🎯 Requirements

• LMSW or LCSW licensed in North Carolina
• Master’s Degree in Social Work or related field
• 1-5 years licensed clinician experience
• Fluent in English and Spanish (medical interpretation)
• Proficient with EHRs and Salesforce experience preferred
