📋 Description

• Lead, inspire, and support Home Health Aides and field staff.
• Visit clients in Nassau County; conduct assessments and supervisory visits.
• Develop individualized care plans and oversee their implementation.
• Monitor client needs, service delivery, and changes in condition.
• Supervise, mentor, and coach caregivers in the field.
• Facilitate orientation for newly hired staff; conduct competency assessments.

🎯 Requirements

• Current Registered Nurse (RN) license in good standing in New York.
• Minimum of two years of RN experience.
• Prior supervisory or leadership experience preferred.
• Home care, personal care, or community-based healthcare experience preferred.
• Strong clinical assessment, organization, communication, and problem-solving skills.
• Ability to travel throughout Nassau County for client visits and community engagement.
