Job Description
📋 Description
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, e.g., mobility and ROM, transfer status, balance
- Implement wound care interventions and use evidence-based dressings and topical agents.
- Develop and implement individualized care plans with manual therapy, gait, balance training, and
- Continually assess and revise the PT care plan and collaborate with nursing in multidisciplinary
- Educate clients and family on rehabilitative care and independent living.
🎯 Requirements
- Current Pennsylvania Physical Therapist license.
- Minimum of one year of recent PT experience.
- Graduation from APTA-approved program or AMA-accreditation.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; EMR experience preferred.
