📋 Description

• Virtual Behavioral Health Counseling for older adults via telehealth
• Lifestyle & Behavior Change Support; build sustainable routines around stress, sleep
• Use EMR for documenting patient information and communicating with other providers
• Collaborate with care coordinators, health coaches, and clinicians to develop personalized care

🎯 Requirements

• Active and unrestricted LCSW, LMFT, or LPCC license in multiple states including CA, TX, FL, and/or
• 3+ years clinical experience; 1+ years telehealth with EMRs and communication tools
• Experience working with older adults (65+) or in geriatric health, chronic disease management
• Excellent communication and empathy-driven clinical approach
• Comfortable working independently in a fast-paced startup environment
