📋 Description

• Build empathetic relationships with patients and families
• Educate patients about diagnosis, prognosis, options
• Assess needs, goals, and preferences via advance care planning
• Translate needs into clinical care needs
• Collaborate with nephrology team to coordinate care
• Manage plan of care across settings and transitions

🎯 Requirements

• Active RN license in good standing
• 3+ years ICU or hospice/palliative care experience
• Experience with goals of care and end-of-life discussions
• Able to pivot quickly in a startup environment
• MS Office (Outlook/Teams) intermediate skills
• Remote work capability and dedicated, interruption-free space for PHI/HIPAA
