Licensed Practical Nurse (LPN), Home Health

📋 Description

• Providing skilled nursing and rehabilitative care to adult clients at home on a per visit basis in
• Making home visits to clients in designated territories as part of a multi-disciplinary health care
• Administering medications, wound care, treatments, and procedures per physician orders.
• Monitoring client conditions and reporting changes to Clinical Manager and Client Services Manager.
• Documenting observations and interventions in electronic medical records on a touch pad tablet.

🎯 Requirements

• One year of prior clinical experience as an LPN.
• A current LPN license in Pennsylvania.
• Graduation from an accredited nursing program.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
