Licensed Practical Nurse (LPN)

📋 Description

• Quality, individualized health care management for each individual
• Routine site visits, vitals, assessments, medication administration counseling, document progress
• Health/medical resource for staff; train staff in medical arena
• Audit and review medications
• Ensure compliance with state requirements and policies for medical care and records
• Schedule: Full-time Mon-Fri with occasional travel; on-call rotation 1 week per month

🎯 Requirements

• Indiana Licensed Practical Nurse (LPN) license required
• 1-2 years experience with individuals with disabilities or long-term care preferred
• Skilled, flexible, dependable; able to work with staff, interdisciplinary team, and outside agencies
• Strong verbal/written communication
• Ability to work independently and efficiently
• Organizational and computer skills
