📋 Description

• Provide quality individualized health care management
• Site visits, vitals, assessments, med counseling
• Health resource for staff; train staff in medical arena
• Medication audits and record-keeping
• Ensure compliance with state requirements and policies
• Full-time Monday-Friday with travel as needed

🎯 Requirements

• Current Licensed Practical Nurse (LPN) license in Indiana
• 1-2 years with individuals with disabilities/long-term care preferred
• Strong communication and independent work skills
• Organizational and computer skills
• Background and driving record check acceptable
