Care Manager, LPN (MO / Field-Based)

📋 Description

• Conduct initial in-home clinical assessments and coordinate service activation.
• Identify plan changes as patient needs evolve and document revisions.
• Educate patients, caregivers, and internal teams on care needs, safety, and service requirements.
• Coordinate patient care through timely communication with physicians and care teams.
• Provide ongoing education to support best practices and caregiver competency.
• Monitor care quality and regulatory compliance; escalate issues per protocols.

🎯 Requirements

• Diploma from an accredited nursing program; LPN license in Missouri in good standing.
• Minimum 1 year of experience caring for elderly or disabled individuals with complex conditions.
• Valid driver’s license and reliable transportation.
• Strong customer service orientation, empathy, and excellent communication skills.
• Ability to follow scripts, workflows, and regulatory requirements; proficient with CRM tools.
