📋 Description
- Conduct initial in-home clinical assessments and coordinate service activation.
- Identify plan changes as patient needs evolve and document revisions.
- Educate patients, caregivers, and internal teams on care needs, safety, and service requirements.
- Coordinate patient care through timely communication with physicians and care teams.
- Provide ongoing education to support best practices and caregiver competency.
- Monitor care quality and regulatory compliance; escalate issues per protocols.
🎯 Requirements
- Diploma from an accredited nursing program; LPN license in Missouri in good standing.
- Minimum 1 year of experience caring for elderly or disabled individuals with complex conditions.
- Valid driver’s license and reliable transportation.
- Strong customer service orientation, empathy, and excellent communication skills.
- Ability to follow scripts, workflows, and regulatory requirements; proficient with CRM tools.
