Home Health RN

📋 Description

• Provide skilled, compassionate home health nursing per CMS guidelines
• Coordinate care plans with the interdisciplinary team (IDT)
• Deliver and document nursing interventions, maintain safe home environment
• Respond to physician orders and administer medications as ordered
• Educate participants, families, and caregivers on disease process and self-care
• Track home health scheduling with Home Health Manager

🎯 Requirements

• Graduate of an accredited school of nursing
• Unencumbered RN License
• Minimum 1 year nursing experience with frail/elderly population; home health preferred
• Reliable transportation
• CPR and First Aid within 180 days of hire; preferred at hire
