• Guide Wellthy families through caregiving logistics and needs.
• Develop comprehensive care plans and oversee ongoing needs.
• Coordinate medical appointments and research providers.
• Track medications, manage medical bills, and assist with insurance/vetting.
• Collaborate with Care Advisers to ensure seamless service.
• Support knowledge sharing and represent Wellthy externally when needed.

🎯 Requirements

• Bachelor’s degree in Social Work, Public Health, Healthcare Administration or similar.
• 4-6+ years healthcare experience (case management, crisis intervention).
• Empathetic, clear communication about healthcare topics.
• Experience developing and facilitating care plans.
• Ability to take feedback and adapt care plans innovatively.
• Capable of handling up to 40 care projects concurrently.
