Licensed Social Worker - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
• Identify appropriate community resource referrals to address practical and environmental needs.
• Advocate for client and family to navigate the patient through the community system.
• Participate in development of the total plan of care with BAYADA clinicians.
• Develop and maintain contracts with public and private agencies as resources for the patient.

🎯 Requirements

• A Master’s Degree in Social Work
• Licensed Social Worker (LCSW or LICSW) in the State of Colorado
• At least one year of experience working independently as a Licensed Social Worker
