Community Health Worker - Bakersfield

📋 Description

• Support members with complex medical and social needs to manage health and wellbeing.
• Motivate behavioral changes through education, peer support, and shared experiences.
• Coordinate with clinical teams to develop and carry out care plans.
• Document information from encounters in designated information systems.
• Assist enrollment into Akido ECM Program and connect to services.
• Travel 50-75%; requires valid driver's license and reliable transport.

🎯 Requirements

• High School Diploma or equivalent.
• Bilingual in English and Spanish strongly preferred.
• 2+ years experience with CalAIM populations (homelessness, SMI/SUD, post-incarceration, complex
• Ability to work outdoors, independently and in teams, manage competing tasks.
• Excellent communication and interpersonal skills; proficient in documenting in case management
• Compensation: hourly, with pay range $22-$25 USD.
