Remote Nurse Care Manager - Mountain Time

📋 Description

• Manage an assigned patient panel and address each patient’s needs within value-based care.
• Collaborate with physicians, APPs, and IDT members on care plans and program goals.
• Support patients in programs like Chronic Complex Care, Transitions of Care, CKD Management.
• Work remotely to support Mountain Time Zone patients; hours 8:30a-5p MT.
• Engage in education, transitions of care, and care plan optimization with the care team.

🎯 Requirements

• Associate degree in nursing
• Current RN license; compact license preferred
• Care management experience required
• MS Office (Outlook/Teams) familiarity
• Ability to work in a remote environment with dedicated space and reliable home internet
