Clinical Pharmacist (NH-licensed)

📋 Description

• Provide integrated support to patients and physicians to improve care.
• Develop personalized relationships to enhance medication adherence.
• Review patient care notes for prior authorization and care plans.
• Monitor adherence across sites and disease states (oncology, specialty).
• Evaluate EMR for drug interactions, labs, allergies, orders, and communications.
• Recommend alternative regimens per guidelines and review therapy for side effects.

🎯 Requirements

• Doctor of Pharmacy (PharmD) or equivalent; must be licensed Pharmacist
• 1+ years experience preferred
• Strong knowledge of specialty meds and regulations
• Proficiency in MS Office; EPIC/QS1/WILLOW a plus
• Ability to improve medication adherence and outcomes
