Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: ROM, transfer status, balance, gait, strength
• Develop and implement individualized care plans: manual therapy, gait training, balance, other.
• Continually assess and revise the PT care plan; collaborate with nursing on care plans.
• Educate clients and families on rehabilitative care and independence.
• Accurately document observations and interventions using EMR.

🎯 Requirements

• A current DE Physical Therapist license (DE PT license).
• Minimum of one year of recent PT experience.
• Graduate from an APTA-approved program or CAAHEA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
