Physical Therapist, Senior Living Communities

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, gait, balance, strength, endurance
• Provide wound care and integumentary interventions with evidence-based dressings.
• Develop and implement individualized care plans with manual therapy and gait training.
• Assess and revise PT care plan; collaborate with nursing in the multidisciplinary team.
• Educate clients and families to promote health, safety and independence.

🎯 Requirements

• A current New Jersey Physical Therapist license.
• Graduation from an APTA-approved program or AMA-equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem solving skills.
• Solid computer skills; EMR experience preferred.
