📋 Description

• Develop and implement nutritional plan of care for participants with social and medical needs, in
• Complete nutritional assessments and reassessments; screen nutritional status and plan nutrition
• Provide nutrition education to participants, caregivers, and Habitat PACE staff.
• Plan follow-ups and reassessments when participant status changes; adapt plans accordingly.
• Assess food access and make referrals for meal prep and grocery support.
• Coordinate with care team and support care planning for home-based participants.

🎯 Requirements

• Master’s degree or equivalent in food science, nutrition, and/or dietetics required by
• Nationally Certified as a Registered Dietitian Nutritionist (RD/RDN).
• Minimum 1 year experience working with frail or elderly individuals; experience with PACE/ADHC or
• Ability to conduct home visits and coordinate with PCP and interdisciplinary team; strong
• Strong learning mindset, data-informed decision making, and ability to adapt in a fast-paced

🎁 Benefits

• Base compensation: $83,000 - $103,000 per year with a target bonus of 5% of annual salary
