📋 Description

• Start of Care/Oasis Admissions RN for Jacksonville, FL
• Per visit basis service across Duval, Baker, Clay, Nassau Counties
• Travel to client homes within designated territories
• Follow designated care plans and administer medications
• Conduct client assessments and document care interventions
• Coordinate case management and document outcomes

🎯 Requirements

• FL Registered Nurse license (current)
• Minimum 1 year clinical (medical/surgical) exp.
• Home health exp preferred; not required
• Strong interpersonal skills and independent work style
• Solid computer skills; EMR experience preferred
• Ability to travel to cases as assigned
