📋 Description
- Per Diem On-Call RN for evening home health coverage in Kent County, DE
- Provide in-home skilled nursing visits and Start of Care admissions as needed
- Deliver patient-centered care with support from an clinical team
- Respond to after-hours patient needs within coverage area
🎯 Requirements
- Current RN license in Delaware
- Minimum 1 year recent Home Health nursing experience
- Experience with OASIS documentation and Start of Care visits preferred
- Strong assessment, decision-making, and communication skills
- Comfort with EMR; HCHB experience a plus
- Reliable transportation; willing to travel throughout Kent County, DE
