📋 Description

• Provide outpatient home health visits for adults in senior living settings.
• Perform outpatient therapy evaluations to develop a plan of care.

• Collaborate with home health therapy partners to coordinate transitions to outpatient services.
• Educate clients, families, and caregivers to maximize functional independence.

🎯 Requirements

• Pennsylvania Occupational Therapist license (current).
• Minimum of one year as an Occupational Therapist.
• Strong time management and independent work skills.
• Excellent interpersonal, organizational, and problem-solving abilities.
• Solid computer skills; EMR experience preferred.
