Physical Therapist- Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfer status, gait, balance, safety.
• Implement wound care interventions with evidence-based dressings.
• Develop and implement individualized care plans: manual therapy, gait, balance, other functions.
• Continually assess and revise the PT care plan with nursing in the care team.
• Educate clients and families to promote health, safety and independence.

🎯 Requirements

• Pennsylvania Physical Therapist (PT) license.
• Minimum one year recent PT experience.
• Graduation from an APTA/AAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
