Physical Therapist (PT), Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, ambulation, strength, balance, safety).
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and implement individualized care plans with manual therapy, gait training, balance
• Assess and revise the PT care plan; collaborate with nursing in multidisciplinary plan.
• Educate clients and families to promote health, safety, and independence.

🎯 Requirements

• Active Pennsylvania Physical Therapist license.
• At least one year of recent PT experience.
• Graduate from APTA/CAAHEP-accredited program or equivalent.
• Ability to work independently and manage time; strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.
