📋 Description
- Clinical Assistant (LPN or CMA) in senior living communities
- Pre-visit planning, chart preparation, and medication reconciliation
- Conduct screenings (depression, cognitive) as part of Wellness Visits
- Maintain and update patient charts; set up e-prescriptions and orders
- Assist providers during field visits and minor procedures under supervision
- Monitor portal messages and support seamless clinical flow
🎯 Requirements
- Active Licensed Practical Nurse (LPN) or CMA/RMA/CCMA certification
- Active driver’s license
- Knowledge of medical terminology and EMR systems
- Strong communication, organization, and patient care skills
- Ability to work in field settings and travel to patient communities
