Registered Nurse - Home Health RN

📋 Description

• Provide skilled nursing in patients' homes as part of a multi-disciplinary team.
• Perform head-to-toe assessments and monitor changes in patient condition.
• Deliver acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care).
• Triages and manages urgent clinical situations in the home setting.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Documentation via point-of-care systems and wound care documentation.

🎯 Requirements

• Massachusetts nursing license in good standing.
• Graduate of an accredited nursing program.
• At least 1 year of hands-on medical experience with adult/geriatric population in relevant settings.
