Care Manager - SNP (Peak Health)

📋 Description

• Coordinate care with Interdisciplinary Care Team (ICT) and providers to ensure continuity.
• Serve as primary care management contact for members and caregivers.
• Support members during transitions of care and conduct virtual visits when needed.
• Provide education and resources to strengthen self-management and outcomes.
• Review HRA data to identify needs and opportunities for targeted programs.
• Manage and triage member self-referrals and coordinate services.

🎯 Requirements

• Current RN license in state of service or multi-state license via eNLC.
• At least 3 years clinical healthcare experience.
• Minimum 1 year in care management, case management, or population health.
• Bachelor’s in Nursing preferred; ASN or diploma considered.
• Experience with Medicare/Medicaid or SNP populations preferred.
• At least 1 year SNP care management experience preferred.
