RN Case Manager

📋 Description

• Lead hospice care with team oversight and clinical leadership.
• Coordinate patient, family, and interdisciplinary team activities.
• Oversee care plans and ensure Medicare compliance.
• Direct documentation, QA, and EMR accuracy.
• Mentor clinicians; participate in meetings with medical director.
• Travel to patient locations for home-based care.

🎯 Requirements

• Bachelor’s degree or higher in a health-related field.
• Minimum 3 years RN experience; hospice experience required.
• At least 1 year supervisory/management in hospice.
• Valid RN license in state of practice.
• CPR/BLS required; Certified Hospice and Palliative Care Nurse preferred.
• Proficiency with Outlook, Word, Excel, and EMR systems.
