Home Care Coordinator

📋 Description

• Conduct in-home care assessments and set framework for home care team
• Coordinate participant care with IDT and multiple providers
• Schedule and track home care services and staffing hours
• Engage with participants, physicians, and family members by phone
• Support enrollment and marketing efforts for the program
• Participate in end-of-life care coordination and support

🎯 Requirements

• Healthcare/medical licensure or equivalent plus 3+ years of experience
• Bachelor’s degree preferred
• Minimum 3 years of case management or nursing in clinical/home setting with frail/elderly
