Clinical Pharmacy Liaison PTCB or NHA Certified

📋 Description

• Provide full suite of medication management services to hospital patients
• Collaborate with hospital clinicians to serve patients face-to-face and by phone
• Educate patients on hospital services and overall care improvement
• Support outpatient retail and specialty prescription needs
• Grow specialty pharmacy patient population
• Manage communications with the centralized Patient Service Center

🎯 Requirements

• State-registered pharmacy technician
• Nationally certified as a CPhT preferred but not required
• 2+ years as a Pharmacy Technician; 3-5 years preferred
• High School Diploma or GED required
• Knowledge of transplant/hepatitis C/infectious disease/oncology a plus
• Strong Microsoft Office skills; excellent written/verbal communication
