📋 Description
- Provide remote pharmaceutical care services for the Therapy Management program
- Conduct phone-based disease state and drug therapy education assessments
- Identify, resolve, and prevent medication-related problems to overcome adherence barriers
- Collaborate with healthcare professionals to develop and monitor medication therapy plans
- Monitor drug interactions, adverse effects, and therapeutic outcomes
- Maintain accurate patient records and contribute to clinical protocols
🎯 Requirements
- PharmD and NY Pharmacist license required
- Strong clinical knowledge in TM program areas (diabetes and/or weight management)
- Excellent communication and interpersonal skills
- Ability to work as part of a healthcare team
- Attention to detail and commitment to patient safety
- Continuing education and professional development commitment
