Job Description
📋 Description
- Provide OT home visits for adults/geriatric patients
- Assess ADLs/IADLs, strength, balance, mobility
- Develop and modify treatment plans
- Collaborate with interdisciplinary team
- Educate patients and caregivers on home safety
- Complete EMR documentation and progress notes
🎯 Requirements
- OT license in New Jersey
- Minimum 1 year OT experience
- Home Health experience preferred
- Graduate of accredited OT program
- Strong assessment, organization, and communication
- Ability to travel across Monmouth, Middlesex, Somerset, Union
