Licensed Social Worker (Westlake, OH / Field-based)

📋 Description

• Hybrid full-time role with required fieldwork and occasional travel to FreedomCare office in
• Oversee care for patients in the Structured Family Caregiving (SFC) program within Ohio. Act as
• Provide ongoing support to patients and caregivers; conduct home assessments and monitor care plans.
• Maintain documentation, ensure compliance with HIPAA and state/federal policies, and coordinate
• Engage with patients and families to interpret agency philosophy and objectives.

🎯 Requirements

• Valid and active Ohio Licensed Social Worker license (CSWMFT Board).
• Bachelor’s or Master’s in Social Work from CSWE-accredited program.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities; home care
• Experience with CRM (Salesforce is a plus); bilingual in Spanish is a plus.
• Valid driver’s license with insured vehicle for travel within the state.
