Licensed Social Worker - Part Time (Rocky River, OH / Field-based)

📋 Description

• Provide oversight of patient care within the Structured Family Caregiving (SFC) program in Ohio.
• Serve as escalation point and advocate for quality of service with FreedomCare and the SFC program.
• Conduct initial in-person home assessments with patient and caregiver.
• Perform day-to-day monitoring and monthly check-ins on care delivery and caregiver support.
• Enroll patients and coordinate credentialing, claims operations, and prior authorizations as needed.
• Ensure compliance with HIPAA, state and federal policies, and agency procedures.

🎯 Requirements

• Valid and active Ohio Licensed Social Worker license (CSWMFT Board).
• Bachelor’s or Master’s degree in Social Work from CSWE-accredited program.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities.
• Home care experience and bilingual Spanish a plus; CRM experience (Salesforce a plus).
• Valid driver’s license with insured vehicle for travel within the state.
