📋 Description Part-Time Social Worker to support elderly participants in PACE Coordinate with the IDT to ensure assessments and care plans are followed Deliver social work interventions, resources, and case management per care plan Support care transitions between settings and participate in care conferences Provide discharge planning when participants disenroll 🎯 Requirements Master’s degree in Social Work (MSW) Minimum 1 year experience with frail/elderly populations Experience coordinating care conferences Medical clearance for communicable diseases; up-to-date immunizations CA driver’s license, transportation, and insurance as required by law CPR/BLS certification or ability to obtain within 30 days 🎁 Benefits Competitive benefits package including medical, dental, vision 401(k) with match and other voluntary benefits Paid time off, holidays, and parental leave Employee Assistance Program and mental health resources
