📋 Description Part-time Physical Therapist providing evaluation and treatment across care levels for PACE Develop individualized treatment plans; conduct home visits as needed. Educate participants and caregivers on exercise programs and DME use. Contribute to IDT care planning, maintain regulatory documentation. Support PT program standardization, team education, and quality audits. 🎯 Requirements Bachelor’s Degree in physical therapy or higher (DPT preferred); current California PT license. Completion of an APTA-accredited PT program. Active CPR/BLS certification; ability to perform home visits and DME guidance. 2+ years of PT experience in geriatrics or with frail/elderly populations preferred.
