Occupational Therapist, (OT) - Home Health

📋 Description Make home visits to clients in designated geographic territories. Evaluate patients’ functional status and ADL/IADL performance. Assess cognitive, psychological and motor aspects; develop intervention plans. Evaluate home environment for hazards for independence. Identify equipment needs and environmental adaptations. Revise care plan with nursing in the multidisciplinary team. 🎯 Requirements Current North Carolina Occupational Therapist license. One year of OT experience preferred; home care a plus. Accredited OT program completion as required. Ability to work independently and manage time. Strong interpersonal, organizational, and problem solving skills.
