📋 Description Perform thorough evaluations of patients’ functional status and home safety for independent living. Assess cognitive, psychological, and motor aspects to develop individualized intervention plans. Evaluate home environment for hazards and barriers to independence. Identify equipment needs and environmental adaptations to enhance outcomes. Update and revise the OT care plan and collaborate with nursing in the multidisciplinary team. Educate patients and families on rehabilitative care to promote health and safety. 🎯 Requirements Licensed Occupational Therapist in Hawaii. Willingness to travel within assigned territory.
