📋 Description Occupational Therapist to join Habitat Health at our South LA PACE Center in a full-time capacity. Deliver evaluation, treatment planning, and direct therapy to support older adults in home and Collaborate with interdisciplinary teams to provide comprehensive care aligned with PACE model. Provide documentation and participate in program meetings to ensure quality care. Assist with ADLs, mobility, and home-based interventions as needed. 🎯 Requirements A minimum of 2+ years as a practicing OT. National Certification by the American Occupational Therapy Association or eligible for National Ability to work in healthcare, community, and/or home-based environments. Strong communication with participants, caregivers, and team members in person/phone/electronic Bilingual Spanish/English strongly preferred; other languages a plus. 🎁 Benefits PACE experience is nice to have (ADCH/CBAS experience acceptable).
