Clinical Manager

📋 Description Lead and coordinate patient care in a structured home health setting. Own caseload planning, referrals review, and clinical order approvals. Facilitate case conferences and cross-disciplinary communication. Ensure timely assessments and alignment with billing guidelines. Supervise field staff and support scheduling and on-call rotation. 🎯 Requirements RN with at least 1 year of recent clinical experience. At least 1 year of dedicated Home Health experience. Strong knowledge of Home Health operations and Medicare CoPs. Proficient in Homecare Homebase (HCHB) software.
