CA Licensed Bilingual Spanish/English Clinician - LCSW or LPCC - Behavioral Health

📋 Description Provides care via the Collaborative Care Model to pediatric and/or adult patients. Tailors treatment plans to individual patient needs. Coordinates behavioral health care with PCPs and other providers. Uses PHQ-9, GAD-7, CSSRS to screen for issues. Monitors symptoms and side effects to reduce symptoms. Documents progress in Salesforce and EHR for sharing with care team. 🎯 Requirements Master’s Degree in Social Work, Counseling, or related field. Licensed in CA as an independent clinician (LCSW/LPCC). Minimum 1-5 years experience as licensed professional. High-speed internet for video conferencing and cloud systems. Proficient in English and Spanish medical interpretation and documentation.
